The Cellular Therapy Discharge Navigator is a registered nurse and a critical member of the Cellular Therapy team, responsible for ensuring that patients receive comprehensive and seamless care following stem cell transplant, CAR-T therapy or post chemotherapy. This includes coordinating frequent follow-up treatments and appointments, monitoring for complications, and providing education and support to patients and their families. This position also requires coordination of financial and insurance-related matters. The coordinator will work closely with the patient's multidisciplinary cellular therapy team and other healthcare providers and/or specialists to ensure continuity of care throughout the treatment continuum. Additionally, the coordinator will provide emotional support, counseling, and assistance to accessing community resources for patients and their families as needed to help them navigate their treatment continuum.

The successful candidate:

  • Must be able to work ONSITE Monday-Friday, 8:00a-5:00p.

  • Monitor patients for transplant, CAR-T, and chemotherapy-related complications, recognizing and addressing issues such as infections, graft-versus-host disease, and CAR-T toxicities.

  • Coordinate treatments and follow-up appointments across outpatient clinics, infusion centers, and external facilities.

  • Complete prior authorizations for specialty medications in a timely and proactive manner.

  • Facilitate transitions across inpatient, ambulatory, outpatient, and external care settings, arranging home care services as needed.

  • Collaborate with cellular therapy finance specialists and insurance case managers to ensure appropriate authorizations and reimbursement.

  • Educate patients and families on post-transplant care, symptom management, and self-care strategies to improve adherence and quality of life.

  • Provide emotional support and assist patients in navigating the healthcare system and accessing essential resources.

  • Participate in ongoing professional development, training, and research related to cellular therapy and cancer survivorship.

  • Maintain accurate patient records while ensuring compliance with all relevant regulatory standards and guidelines.

  • Serve as a liaison between patients, healthcare teams, support services, and referring organizations to promote seamless care coordination.

  • Assist in the development and implementation of organizational policies and procedures.

  • Support and contribute to the strategic goals of the Cellular Therapy Program at City of Hope Atlanta.

Qualifications

Your qualifications should include:

  • Current Georgia RN license from an accredited college

  • BSN required or enrolled within 6 months of hire and BSN completed within three years from hire date

  • 3 - 5 years Oncology/Hematology experience

  • Basic Life Support (BLS) Certification, Advanced Cardiovascular Life Support (ACLS) certification, ONS Chemotherapy and Biotherapy Provider certification.

Salary / Pay Rate Information:
Pay Rate: $37.81 - $62.28 / hour

The estimated pay scale represents the typical [salary/hourly] range City of Hope reasonably expects to pay for this position, with offers determined based on several factors which may include, but not be limited to, the candidate’s experience, expertise, skills, education, job scope, training, internal equity, geography/market, etc. This pay scale is subject to change from time to time.
